Abstract:

Objective: Estimate the variation in the prevalence of postpartum depression symptoms (PDS) in Brazil and to examine how this prevalence may have changed across different sociodemographic groups and macro-regions, between 2013 and 2019. Methods: The study used data from National Health Survey, 2013 and 2019. PDS were assessed using a Patient Health Questionnaire-9 score ≥ 10 in adult women who had a child within 18 months prior to the interview. The variation in prevalence between surveys was expressed as absolute change and as the prevalence ratio percentage. Results: There was a shift in the profile of women who had given birth with respect to age group and educational level, indicating that women are having children at older ages (35 years or older) and with higher levels of education (completion of secondary education or more). An increase in the prevalence of postpartum depression symptoms (PPDS) was observed across all categories studied, with a relative increase of 43.3% (95% CI: 6.4;93.1) nationwide, rising from 9.9% (95% CI: 7.8;12.5) in 2013 to 14.2% (95% CI: 11.8;16.9) in 2019. The largest variation was observed among women aged 35 years or older (148.4% - 95%CI: 21.8;406.5). Additionally, a substantial increase was found among those with the lowest level of education—up to incomplete primary education (103.9% - 95%CI: 16.5;256.7). Conclusion: These increases are presumed to be associated with the economic crisis experienced in the country during the period, as well as with changes in Brazilian family structures. The findings highlight the need for public investment in maternal mental health during the postpartum period.
