Abstract:

Introduction: a positive and safe childbirth experience is still a challenge for parturients around the world and also in Brazil. Objective: to describe and analyze changes in perineal care and episiotomy rates in a maternity school in the state of São Paulo, and the strategies that facilitated the incorporation of evidence-based practices. Methods: mixed-methods study that analyzed changes in the care model and episiotomy rates through data from medical records (2010 to 2022), using elements of the Change Laboratory methodology and the historical and cultural analysis of the activity system. Results: the episiotomy rate was 70% in primiparous women and 35% in the general population in 2010, reaching in 2022, 0,7% and 0,5%, respectively, allowing 75% of parturients (65% of primiparous women) to have perineal integrity or first-degree injuries. Contributed to change: the creation of protocols, the monitoring of indicators, mandatory registration of the episiotomy, team training, reduction in the use of forceps and strengthening of communication strategies with parturients. Conclusion: a drop in the episiotomy rate is possible even in teaching hospitals, as long as training is based on evidence and human rights.
